Terms and Conditions for the Rating Campaign

1. Introduction

These Terms and Conditions govern participation in the Rating Campaign, an initiative designed to encourage customer engagement through feedback and social media interactions.

2. Eligibility

Open to individuals aged 18 and above, with no geographical restrictions. Entry is limited to one Google review per person.

3. How to Enter

Participants can enter the campaign by scanning the QR code and engaging with any clinic under the Careplus Group or Chye Clinic. Limit of one entry per person per location.

4. Selection of Winners

The draw occurs every 4 months as outlined below, with winners selected randomly from eligible entries:

• Period 1: 1/12/23 - 1/5/24 (winner announced on FB/IG)
• Period 2: 2/5/24 - 07/9/24 (winner announced on FB/IG)
• Period 3: 22/11/2024 - 25/3/2025 (winner announced on FB/IG)
• Period 4: 1/5/2025 - 20/9/2025 (winner announced on FB/IG)
• Period 5: 7/10/2025 - 7/2/2026

5. Prizes

Period 1: 3 Days 2 Nights Stay at Grand Hyatt Hotel KL

Terms and Conditions:

- Not applicable for public holidays.
- Cannot be combined with other promotions.
- Hotel stay rules strictly follow Grand Hyatt Hotel KL.
- Cannot be redeemed as cash.
- It can be exchanged for another person as a gift.

6 .Winner Notification and Claiming Prizes

The winner will be announced on our social media platforms, Facebook and Instagram (careplusclinic.my). Please stay tuned and follow us for updates

Winners are required to follow the sponsor’s business and post about their lucky draw win on social media.

8 .Sponsorship

Businesses can become sponsors with promotional benefits.

9 .Data Protection and Privacy

Participants need to submit their email address, name, and contact number for lucky draw notifications and email marketing campaigns.

10. Liability and Disclaimer

This campaign is not liable for any damages or losses resulting from participation. The lucky draw is subject to all applicable laws and regulations.

11. Changes and Termination

Careplus Group/Chye Clinic reserves the right to modify the Terms and Conditions or terminate the lucky draw at any time without notice.

12. Governing Law

This lucky draw is global, and participants agree to comply with applicable international laws.
